We invite you to take part in a European-wide survey to gain a picture of the Physical Health problems experienced by people suffering from a mental illness. We would like to invite you to take part in this survey. This study is looking at the experiences of adults (aged 18 years or older) who have been diagnosed with a mental illness. If you are less than 18 years of age or do not have a diagnosis of mental illness then please do not complete the survey. If you are suitable to take part then it is entirely up to you whether you decide to do so or not. The survey is anonymous so nobody will know whether you decide to take part.

By taking part, you will be involved in research which will help us to understand the frequency and nature of Physical Health Problems experienced by people with a diagnosis of mental illness.
